Job description:

The Companion is responsible for providing competent companion and homemaking service to patient/clients in accordance with the policies of the agency and care plan of the patient/client under the direction of a licensed nurse. Starting wage is dependent on prior experience.

Duties:

• Perform light household tasks such as planning and preparing meals, laundry, making bed, washing dishes, dusting, vacuuming, and shopping.
• Assure safety and provide minimal assistance in activities of daily living.
• Assist families to maintain their normal and usual activities of life.
• Provide patient/client care: read or write letters; play games or cards; engage patient/clients in conversation; orient patient/client to person, place, date and time; and provide emotional support.
• Provide stand-by assistance with physical activities: accompany patient/client to medical appointments, on errands; assist with ambulation indoors or outdoors; provide stand-by assistance for patient/client transfer to chair, bed, and commode.
• Exercise initiative and judgment in performance of duties.
• Maintain charting and records as required by agency policy and federal and state regulations, assuring that charting follows the plan of care established by the case manager.
• Maintain skills through attendance at Annual Skills Fair or other continuing education opportunities.
• Participate in special projects and perform other duties as requested.
• Keep a daily record of time worked per agency policies.

Education & Experience:

• graduation from high school or completion of GED
• basic level knowledge of EMR (electronic medical records) and email functions.
