Person Centered Case Management Certification required
*Responsibilities: *
The essential function of the case manager is to work in partnership with persons served toward individual goal attainment based upon specific objectives identified in the treatment plan. The approach is strengths based and the majority of service is provided in the community setting. Case managers must be able to transport clients. Some availability after working hours is required.
*Job Requirements:*
This position requires: a person with Person Centered Case Management Certification from the Kansas Department for Aging and Disability Services (KDADS) or a licensed addictions counselor (LAC), with the ability to obtain Person Centered Case Management Certification within six months of employment. Experience with persons with chemical dependence and/or severe and persistent mental illness is preferred. Ability to work flexible hours as needed. A valid Kansas drivers license and access to reliable personal transportation and necessary insurance is required. Must be at least 18 years of age or older and be able to pass pre-employment background checks and KDADS Child Abuse and Adult Abuse Registry checks. This position requires knowledge of community services in Topeka and basic computer skills and a working knowledge of Microsoft Office Excel.

Pay: $21.97 - $24.27 per hour
